My son has an ear infection and is having trouble hearing.
Should I worry?

Q) Dear Dr. Gwenn:
My 4 year old son recently got the flu with a double ear infection. He's on an antibiotic and has 3 days remaining. He is much better except he's complaining of trouble hearing. I've actually found him standing very close to the TV despite a reasonable volume level. Does he need another antibiotic? Should I be worried about this recent hearing loss issue? Should I get his hearing checked?

A) Dear Tara:
This is a very common problem during an acute ear infection because fluid builds up in the middle ear and alters the perception of sound. It's like trying to hear while underwater. The fluid can persist for 2-3 months after an ear infection and for most kids any hearing loss is temporary and resolves as the fluid resolves.
Since your son's major symptoms are improving, a new antibiotic is not needed. However, sometimes when fluid lingers, a new ear infection can develop so be sure to have your son's ears checked if the ear pain or fever returns.
A hearing test is very useful especially if your son's hearing doesn't improve over the next few weeks. But, especially since 4 year olds can have trouble sitting still for a hearing test, you may want to talk to your pediatrician about waiting a few weeks for the fluid to resolve. Having his hearing checked now may not be the best timing but could be useful as a baseline.
Finally, while this sounds illness related, 4 year olds are notorious for tuning out their parents at home. Before you worry too much, talk to your son's preschool teachers and see if they notice any hearing issues. The problem may not be that severe if he is hearing well enough to get through school or playdates.
Time and statistics are on your side for this to be a very temporary blip in your child’s life. So, be vigilent but don't worry too much.
